Forms

These are forms for homeowners' use to request architectural change authorization, guest parking permits, or clubhouse rental. Each form is dealt with differently. The procedure for each is given below and with the form.

Architectural Request Form: Use this form to request authorization for changes, such as patio cover, security door, or solar tubes.
Before making a modification to your home, check with the Management company. If you're advised that you need authorization to make a desired modification to your home, then this is the required form. There are instructions on the form, and the Management company can give you more information. Print this form from your computer, complete the form as indicated, and send it by postal mail to the Management company. This form is in PDF format, and requires the Adobe pdf reader to view and print. If you do not have the Adobe pdf reader on your computer, go to Adobe and follow their link to "Get Adobe Reader" to download and install it.
Guest Parking Permit: for use of Guest Parking spaces
You may request a parking permit for your guest by e-mail, telephone, fax, or postal mail. To request via e-mail, use this Guest Parking Permit Request Form. Provide the information by typing it into the spaces provided in the body of the form and click the "Send this Form" button. If all the necessary information is included, you will receive a Guest Parking Permit in the postal mail. To request a permit by phone, fax, or postal mail, contact the Management company with this information:
Clubhouse Rental
The Clubhouse Rental Agreement Form has information about responsibilities of the renter and payments. This form is in PDF format, and requires the Adobe pdf reader to view and print. If you do not have the Adobe pdf reader on your computer, go to Adobe and follow their link to "Get Adobe Reader" to download and install it. You will need two copies of the printed form; one for you and one for the HOA Clubhouse manager. Rental fees vary depending upon when you wish to use the Clubhouse. Call the HOA Clubhouse manager for availability of the Clubhouse. Consult your most recently mailed HOA Newsletter for the name and phone number of the HOA Clubhouse manager. Or, ask the Management company.
